The ST4 selection for Respiratory Medicine is the national recruitment process for higher specialty training in respiratory (and dual General Internal Medicine) post-IMT/CT. It includes a structured interview assessing clinical reasoning, service knowledge, leadership, and commitment to the speciality.
In the 2024 round, there were 367 applications for 109 posts (competition ratio ~3.37:1). Competition rose slightly in 2025 with ~413 applications for 104 posts (~3.97:1).
Successful preparation includes: understanding the higher specialty training pathway; practising scenario-based questions (e.g., service redesign, leadership in respiratory care, complex case discussion); structuring answers with clear frameworks; refining your portfolio narrative; and rehearsing timed responses.
